The following forms are required when enroling your child at St Michael's Catholic School. They are here for your convenience to print out. Alternatively, you may collect an enrolment pack from the office which will also contain these forms.

The Data Collection form collects information to enable nationally comparable reporting of students' outcomes. Download Data Collection Form
The Responsible Use Forms indicate that the students and parent/guardian are aware of acceptable computer/technology use procedures. There are three forms which need to be signed to acknowledge the parent's and student's understanding of acceptable use.
